About Insurance & Payment
Insurance plans that include preventive and out-of-network benefits may reimburse you for our service costs. We'll provide you a detailed invoice to assist you with your insurance claim.
To learn about your out-of-network benefits*, consider posing these questions to your health insurer:
Do I have out-of-network benefits (or only in-network benefits)?
If yes:
1.) Do these benefits cover preventive services, ie CPT code 99386 (preventive medical evaluation)? 2.) What percent of such service charges are typically applied towards the out-of-network deductible (or reimbursed to me once the deductible is met)? 3.) What is my out-of-network deductible and have I met it this annum? 4.) Am I required to obtain a doctor’s prescription to obtain out-of-network coverage?
If no:
Health Sharing, Flexible Spending Accounts and Health Savings Accounts typically cover 100%.
*Medicare does not cover non-Medicare providers. Supplemental plans may provide coverage. For instance, some Physician’s Mutual plans cover 100% of preventive services outside of Medicare providers.
Insurance Coverage
While mainstream medicine and insurance cover diagnoses and treatment, thermography supports prevention. While some insurers will reimburse our services costs, The Thermogram Center, Inc. is a fee-for-service provider, accepting credit card and cash payments.
Payment
A credit card deposit is collected upon scheduling with any remaining balance due collected during imaging appointments via credit card and/or cash.
Rescheduling
Rescheduling is permitted, but PLEASE alert us with at least 48 business-hours notice so we may have time to fill your vacancy. (We understand emergencies may occur, i.e. illness or injury, death of a loved one.)
Cancelling/Refunds
If you pay a deposit or in full and cancel without rescheduling with more than 48 business-hours' notice, you will be refunded the paid amount less the 3.5% credit card processing fee (93% refund if by credit card and 96.5% refund if by mailed check).
If you pay a deposit or in full and cancel without rescheduling with less than 48 business-hours' notice, the deposit amount will be forfeit. For instance: $100 deposit for breasts or $200 for breasts, abdomen, back and face.
